Its all there on voip-info.org. search for realtime architecture. However you'll need some GUI to interact with the MySQL database. When using realtime, you don't deal with voicemail.conf, and will need to write a dialplan which could interact with the database. It'll need some learning though, but its not very hard for a simple setup and voip-info.org has all the info for it, that's where I learned it from.
