You have your standard dialplan, usually something like:
exten => _X.,1,...
You just put this in:
exten => _+.,1,Goto(00${EXTEN:1},1)
And poof, all numbers starting with + get it replaced with 00.
You may need to do more munching of course, and 00 may not be right
for you.
