Have you tried setting the CALLERID variables? If the provider is ignoring those then I guess they are asking you to set per call blocking? I don't know how to do that.

exten => s,1,Set(CALLERID(number)=3025551212|a)
exten => s,n,Set(CALLERID(name)=Joe Smith|a)
