We just experienced this ourselves. First our second module stopped working, then our fourth. We popped an old Sangoma card in and the lines worked fine, so I figure it must be the modules. I'm going to swap the modules in a tester box soon and see if the suspected faulty ones light up. If they're still not working, I'll call Digium. They're under a two year warranty, and I've heard it's usually not a problem to get replacements.
