Stupid not-quite-an-answer - if you're willing to pay money for a fix, why not buy an IAX2 compatible FXS to replace the SPA-1001 with? It will traverse NATs without a problem.
Looks like it wasn't a NAT of configuration problem after all... the SPA devices are quite nice, IMO.
