As a clarification for further posts, its wise to delete both the asterisk modules and header directory when having problems upgrading from 1.0 to 1.2 and depracated modules are in the way. 

As Rob T. pointed out the best way to do this is:
# mv /usr/lib/asterisk/modules /usr/lib/asterisk/modules.old
and
# mv /usr/include/asterisk /usr/include/asterisk.old

/usr/lib/asterisk/modules/app_md5.so is a leftover from your previous installation.
