A thought: I had similar problems with one phone of mine after I power-cycled it during the provisioning process. Its a known issue with the Polycoms that they can become.. confused.. if power-cycled while they're booting.

Have you tried booting the phone offline and formatting its filesystem via the Advanced menu?
