The License Usage Report was introduced in AR System 7.1.00 - which is one 
reason why you may be having difficulties finding it.  The License Usage report 
in 7.1.00 and later doesn't use a form anyway - it uses a log file kept on the 
server.

The ratio of fixed to floating varies depending on the environment, but 
typically it is expected that one floating licenses is equivalent to 2.5-3 
floating licenses (based on 3 work shifts of 8-9 hours each in a 24 hour 
environment).

Licenses are described in the Concepts and Configuration Guides.

A single AR System user licenses can serve as the pre-requisite for multiple 
application user licenses _if used by the same person_.  So if you have 10 
Change User and 10 Help Desk User licenses *and* 10 human beings using both at 
the same time, you'd need 10 AR System User licenses.  If you have 10 Change 
Users and 10 *different* Help Desk Users (i.e. 10 different human beings), then 
you'd need 20 AR System User licenses since you have 20 human beings that may 
be logged in at any given time.

Hello,
I am trying to generate license usage report on our server but I do
not see the license usage form.
I have couple of question on the same:
1. How to generate license usage report(environment information
below)?
2. How to understand how many licenses (fixed/floating) we actually
need?
    Is there a specific ratio fixed:floating that BMC recommends.
3. Are there any supporting docs which I can refer to understand
licenses in detail?
4. How does AR User Fixed, Change User, HelpDesk User Fixed relate to
each other?
    If I currently apply/have 10- Change User Fixed and 10 = HelpDesk
User Fixed, does that mean I need to apply 10+10=20 AR User Fixed
Licenses?
