Open the form i RA and check all unique indexes, you have at least two records 
in the database with the same values in the fields included in the index. An 
easy way to find those entries are to export the fields to excel and sort the 
columns, add a formula checking if row x is similar to row x-1, after you have 
changed those entries to unique combinations, you will be able to add a new 
unique index.
