Thanks to all who helped. Turned out there were some home grown triggers in the database getting in the way. SQL logging pointed me in the right direction.

I have my duplicate preferences set to "Update Old Record with New Records Data." I STILL get a unique index error (ARERR 382). There are no user defined unique indexes on our Change form. My filter logs don't show a push to another form. Could this be coming from the Change ID (1) despite my duplicate preferences?
