just extract the data you need from the num array, something like this will
probably do:
$scope.submit = function(isValid){
if(isValid){
var data = {}
angular.forEach($scope.num,function (el){
data[el.model] = el.modelValue;
});
console.log(data);
//$http.post('/shifts/create', $scope.form)
//.success($state.go('shift', {}, {reload: true}))
}
}
