You don't, it's automatic (as long as you don't exclude countries in the
Market console).
Declined transactions are a frequent occurrence (I see about 10% for
in-app billing), often with the wonderfully clear message "we could not
process the order in a timely fashion".
2) Google says 4 installs but 0 active installs. But I see that I
successfully processed some orders. Isn't an active install one that
is being used - ergo one that was paid for?
The numbers don't update immediately - when they update at all.
